Required Forms:
Alumni Service Hours Form - This form is to be completed by the student and signed by their high school principal.
Standards and Guidelines for VIRTUS - Please read over before your volunteer date.
Acknowledgment of Standards and Guidelines - This form needs to be signed by both the student and parent to maintain on file.
